Abstract: El nacimiento prematuro es un problema de salud pública, al incrementar el riesgo
de complicaciones que pueden comprometer la vida del infante, relacionándose con
infecciones que podrían desencadenar un parto prematuro, incluidas dentro de ellas
la enfermedad periodontal en la gestante. El problema de investigación estudió la
enfermedad periodontal como factor de riesgo y los niños nacidos pretérmino del
Hospital Antonio Lorena del Cusco, donde el objetivo del estudio fue evaluar la
asociación entre la enfermedad periodontal y los niños nacidos pretérmino. Se
realizó un estudio de casos y controles, la muestra estuvo conformada por 70 casos
que fueron las gestantes, quienes desarrollaron trabajo de parto prematuro, con o sin
enfermedad periodontal y 70 controles que fueron las gestantes que no desarrollaron
trabajo de parto prematuro con o sin enfermedad periodontal; todas ellas asistieron
al Centro Materno del hospital. El estudio fue aprobado por el Comité de Ética del
hospital y la selección de la muestra fue por conveniencia, donde participaron las
gestantes que cumplieron los criterios de selección. Se realizaron las evaluaciones
periodontales con el índice CPINTm y se recolectaron los datos médicos y
odontológicos relacionados con las gestantes. Los resultados indicaron una
asociación de la enfermedad periodontal, como factor de riesgo para nacido
pretérmino con un OR= 5.142. Respecto a las características sociodemográficas de
las gestantes; para ambos grupos predominó el grado de instrucción secundario,
para el grupo de casos la procedencia de la zona rural fue 80%, para el grupo de
controles la procedencia de zona urbana fue 70%, el promedio de edad para ambos
grupos fue de 28 ± 4.7 y 27± 4.6. La enfermedad periodontal para el grupo de casos
tuvo un promedio de CPINTm de 2,48 ± 0,60 y para los controles de 1,34 ± 2,48.
Las variables intervinientes que tuvieron efecto sobre la asociación entre la
enfermedad periodontal y el parto pretérmino fueron: el estado civil p=0.018,
procedencia p=0.010, grado de instrucción p=0.001 y el IMC p=0.015. Se concluye
que existe asociación estadística de la enfermedad periodontal como factor de
riesgo para nacidos prematuros.
Preterm birth is a public health problem, as it increases the risk of complications that can compromise the life of the infant, related to infections that could trigger preterm birth, including periodontal disease in the pregnant woman. The research problem studied periodontal disease as a risk factor and preterm infants born at the Antonio Lorena del Cusco Hospital, where the objective of the study was to evaluate the association between periodontal disease and preterm infants. A case- control study was conducted, the sample consisted of 70 cases which were pregnant women who developed preterm labor, with or without periodontal disease and 70 controls which were pregnant women who did not develop preterm labor with or without periodontal disease; all of them attended the Maternal Center of the hospital, the study was approved by the Ethics Committee of the hospital and the selection of the sample was by convenience, where the pregnant women who met the selection criteria participated. Periodontal evaluations were performed with the CPINTm index and medical and dental data related to the pregnant women were collected. The results indicated an association of periodontal disease as a risk factor for preterm birth with an OR= 5.142, with respect to the sociodemographic characteristics of the pregnant women; for both groups secondary education was predominant, for the case group the origin of the rural area was 80%, for the control group the origin of the urban area was 70%, the average age for both groups was 28 ± 4.7 and 27± 4.6. Periodontal disease for the case group had an average CPINTm of 2.48 ± 0.60 and for the controls of 1.34 ± 2.48.
