To analyze the available scientific evidence on the effects of protein supplementation on muscle mass and strength in healthy older adults. Methodology: Systematic review of randomized clinical trials, conducted according to the methodology proposed by the Cochrane collaboration and reported according to recommendations of the PRISMA statement (Preferred Reporting Items for Systematic Reviews and Meta-analyses). The literature search was carried out in Medline, Web of Science, Cochrane Library and LILACS, until November 15, 2022. The risk of bias was evaluated using RoB 1. Results: 13 studies were included that could not be meta-analyzed due to significant heterogeneity (I2 > 50%). Protein or amino acid supplementation showed no significant differences compared to placebo for most evaluated outcomes. In the supplemented groups, two studies reported improvements in muscle strength and another two studies in physical performance. These improvements were observed using milk protein without physical activity, and essential amino acids with or without physical activity. The majority of studies presented low risk of bias. Conclusions: The benefits of protein and amino acid supplementation were inconsistent between studies. The heterogeneity in the interventions and methods of evaluating outcomes limits the ability to establish definitive conclusions. Future studies with similar methodologies are required to determine more precisely the benefits of protein supplementation on muscle mass and strength in healthy older adults.
