Background: Human T-cell lymphotropic virus type 1 (HTLV-1) is endemic in Peru, with evidence of intrafamilial transmission in other countries. In Peru, no studies have been conducted on this issue, and HTLV-1 is not screened for in relatives of positive cases, which is crucial for early diagnosis and prevention. Objective: To determine the prevalence of HTLV-1 infection in a family group following an index case, as well as to determine the frequency of associated diseases, studied relatives, and HTLV-1 prevalence by relationship. Materials and methods: A retrospective study was conducted by reviewing the records of the HTLV-1 Research Group at the Institute of Tropical Medicine. Index cases were identified as the first positive individual in a family group, along with their immediate relatives (father, mother, siblings, children, and partners). The prevalence of HTLV-1 was determined in the relatives of these index cases, adjusting the confidence interval using a cluster estimator. Results: A total of 1,255 index cases and 3,431 relatives were studied. The frequency of studied relatives was 30.30%. The prevalence of HTLV-1 in studied relatives was 36.43% (95% CI: 34.67-38.23%). The HTLV-1 frequency was 63.30% in mothers, 52.33% in fathers, 32.65% in siblings, 22.84% in children, and 57.19% in partners. Conclusions: The found prevalence suggests the presence of intrafamilial transmission in the cohort, making it necessary to expand studies to identify associated risk factors. Additionally, public health policies are urgently needed for early detection in relatives of positive cases.
