Abstract: Antecedentes: Aproximadamente un tercio de pacientes con trastorno depresivo mayor presentan falta de respuesta a la terapia antidepresiva, generándose el concepto de depresión resistente al tratamiento (DRT). Ante esta situación, se ha estudiado la esketamina como alternativa al tratamiento. Objetivo: Evaluar la evidencia disponible acerca de la efectividad y/o eficacia de esketamina como terapia en casos de DRT con respecto a la heterogeneidad de las poblaciones y metodologías de los estudios. Metodología: Se realizó una revisión sistemática exploratoria con los términos clave “esketamine” y “treatment resistant depression”. Se seleccionaron ensayos clínicos que incluyeron desenlaces de eficacia y/o efectividad. Se tomó información sobre la sociodemografía, características de los ensayos clínicos, características de las intervenciones, los resultados y riesgo de sesgo individual. Resultados: Fueron incluidos 13 ensayos clínicos. Se encontraron 10 definiciones distintas para DRT y 3 para remisión. La población incluida fue mayoritariamente blanca, entre 18-64 años y sin comorbilidades médico-psiquiátricas. Tres estudios favorecieron la eficacia de esketamina a corto plazo y cuatro, lo contrario. La vía preferente fue intranasal. Los eventos adversos más frecuentes fueron mareos y disociación. Conclusiones: Hay resultados controversiales sobre la eficacia de esketamina en DRT, pero coinciden en su buen perfil de seguridad. No hay consenso en definiciones de DRT ni de remisión y ante la poca variedad de población, los resultados de los estudios no son generalizables a la población real.
Introduction: About a third part of the patients with major depressive disorder present with non-satisfactory outcomes to the antidepressant therapy, an issue that poses the definition of treatment-resistant depression (TRD). Regarding this problem, esketamine has been studied on the subject. Objectives: To evaluate the evidence regarding the efficacy or effectiveness of the esketamine as a therapy for TRD, regarding the heterogeneity of the included populations and their respective methodologies. Methodology: We conducted a scoping review with the key words "esketamine" and "treatment resistant depression", the clinical trial that included outcomes on efficacy and/or effectiveness were included. We gathered information regarding the demographics, characteristics of the methodology of the clinical trials and their interventions, as well as their published results and individual risk of bias. Results: There were 13 clinical trials included in our review. We found 11 distinct definitions for TRD and 3 for remission. The included populations were mostly white individuals between 18 and 64 years old, without medical or psychiatric comorbidities. Conclusions: There are controversies around the efficacy of esketamine on TRD between studies, but there is an agreement regarding an appropriate security profile. There is still no consensus on the definitions for TRD nor remission. The inability to do a real-world generalization of the result of the studies may be explained in part by the limited number of participants in these studies, which results in a small sample number.
