Malaria is a persistent disease in our country. Although transmission has reduced in the last years, it remains present in regions of the Peruvian Amazon. In Peru, the Loreto department harbours almost all malaria cases, mainly caused by P. vivax. Although we understand the transmission dynamics of the parasite, the role of human mobility associated with malaria transmission, a crucial factor for many infectious diseases, in Peru and globally. This study seeks to examine the role of human mobility in malaria transmission in the Amazonian district of Mazan, Loreto. To achieve this, we worked with seven rural riverine villages scattered along two river basins: the Mazan and the Napo river basins. We found that the population in communities along the Mazan River present sociodemographic characteristics that significantly increase their exposure to the vector compared to those along the Napo river. We also found that human mobility was more common in the population along this basin compared to that on the Napo river. Applying multivariate analysis, we identified characteristics of the mobile population and the conditions significantly associated with a positive diagnosis of malaria by qPCR in these participants. Finally, we explored the connectivity between communities resulting from human mobility through social network analysis. We complemented this analysis with data on malaria cases obtained by DIRESA through passive case detection (PCD). We conclude that human mobility allows the importation of parasites from campsites along the rivers to communities with high mobility profiles and that the local transmission likely keeps this parasite circulating inside the villages.
