Background: In our country there are few studies on the microbiological and resistance profile of biliary tract infections. Therefore, it’s relevant to conduct a study in which these parameters are evaluated to guide antibiotic therapy henceforth in a more effective way. Objective: Identify the bacteriological and resistance profiles of isolates from bile cultures obtained during laparoscopic cholecystectomies and determine the association between clinical characteristics of patients and culture positivity. Methods and Materials: Cross sectional study that evaluated 669 patients’ samples collected during surgery by laparoscopic puncture inoculated in aerobic and anaerobic culture media. After bacteria were identified, antibiotic sensibility was tested. Results: In total, 125 (18.7%) positive bile cultures were obtained. A total of 129 organisms were isolated, most common were E. coli (40.3%), Staphylococcus spp. (24.8%) and Klebsiella spp. (10.9%). No anaerobes were found. 21.2% of E. coli and Klebsiella spp. were ESBL+. Gram negative bacteria had 38% resistance to first generation cephalosporins and 24% to ciprofloxacin. Elevated GGT, the performance of intraoperative cholangiography, and older age were associated to culture positivity. Conclusion: Bacteriobilia was 18.7%. The most prevalent bacteria was E. coli. Prevalence of ESBL+ bacteria was 21.2%. Resistance to first generation cephalosporins was higher than ciprofloxacin. The most important risk factors for bacteriobilia were old age, high values of GGT, and performance of intraoperative cholangiography.
