Objective: To determine the prognostic factors of mortality, prolonged length of stay and functional decline in older patients hospitalized in the Department of Internal Medicine of the Hipólito Unanue National Hospital. Material and methods: Observational, analytical, prospective cohort study. 360 patients older than 60 years, admitted during the year 2019 and evaluated through the Comprehensive Geriatric Assessment were included. The primary combined outcome was the occurrence of mortality or prolonged length of stay. Mortality, prolonged length of stay and functional decline during hospitalization were considered as secondary outcomes. Associations were established by calculating the Relative Risk (RR) with 95% confidence intervals (CI). Adjusted associations were calculated based on a multivariate Poisson regression model. A p value less than 0.05 was regarded significant. Results: Hypoalbuminemia was an independent prognostic factor for the primary combined outcome of mortality or prolonged length of stay (RR=1.37; 95%CI: 1.03-1.83, p=0.028), as well as the secondary outcomes of mortality (RR=3.32; 95%CI: 1.07-10.25, p=0.037) and functional decline (RR=4.63; 95%CI: 1.22-17.68, p=0.025). Functional dependence status was also associated with the main combined outcome (RR=1.51; 95%CI: 1.13-2.01, p=0.005) and with a prolonged length of stay (RR=1.51; 95%CI: 1.14-1.98, p=0.004). Patients with acute confusional syndrome (RR=5.29; 95%CI: 2.39-11.71, p<0.001) and pressure ulcers (RR=1.36; 95%CI: 1.09-1.70, p=0.006), had a higher risk of mortality and prolonged length of stay, respectively. Additionally, cognitive impairment (RR=4.36; 95%CI: 2.06-9.22, p<0.001) and not having a stable partner (RR=2.31; 95%CI: 1.11-4.86, p=0.026), were associated with a significant risk of functional decline. Conclusions: Hypoalbuminemia and functional dependence status on admission were independent prognostic factors of the primary outcome of mortality or prolonged length of stay. Geriatric syndromes such as acute confusional syndrome, pressure ulcers, and cognitive impairment were associated with the secondary outcomes of mortality, prolonged length of stay and functional decline during hospitalization, respectively. Additional studies are required to define the role of social factors in hospital functional impairment.
