The reportability of 1,356 incidents by the employees of the Semaint SRL organization, generates a problem of evaluation of the risk levels, which maintains a standard or statistical deviation and there is a need to design and evaluate a new RACS format to correct the evaluation and build a non-parametric relationship of the high risk level with the type of incidents in the activities of manufacturing, assembly and mechanical maintenance of mining infrastructure of the company Semaint SRL, in a non-experimental, transectional, descriptive-correlational design, with the inductive method; To obtain the results, it was necessary to design and implement a new RACS report format and insert a risk matrix into it, to establish non-parametric statistical correlations between the level of risks and the type of incidents, thus finding the root cause of the problem. The non-parametric statistical result at the post-test level yielded Spearman's Rho coefficients at 99% confidence of: 0.801, 0.820 and 0.770 for the degrees of low, medium and high risk respectively, with a reliability of the research instrument, using the method Cronbach's α of 0.859. The design of the new RACS format has corrected the imperfect evaluation of the level of risks and has established a direct, positive relationship at a 99% confidence level between the type of incidents and the levels of risk in their reportability.
