OBJECTIVE: To analyze the factors associated with mortality in patients admitted to the emergency room due to spontaneous cerebral hemorrhage in the emergency area of the Peru-Korea Friendship Hospital Santa Rosa II-2 Piura in the last 3 years. MATERIAL AND METHOD: A case-control design was performed with a sample of 155 patients, of which 70 patients were left due to inclusion criteria. Clinical variables (Glasgow coma scale, headache), imaging variables (hematoma volume, midline deviation, hematoma location, ventricular invasion) and systemic inflammatory indices (neutrophil/lymphocyte ratio) were analyzed. RESULTS: During a median hospital stay of 30 days, 34 patients (48.57%) died. Patients who died remained in the hospital for a shorter time than those who survived. In multivariate logistic regression, the following parameters were found to be associated with in-hospital mortality: Glasgow Coma Scale score less than 8 points (OR, 15.889; 95% CI, 3.385-74.579; p= 0.000), midline shift (OR, 1.939; 95% CI; p= 0.000), intraventricular extension of hemorrhage (OR, 11.600; 95% CI, 3.582-37.567; p= 0.0001), and neutrophil/lymphocyte ratio (OR, 29.526; 95% CI, 3.636-239.7; p= 0.002). At discharge, 36 patients showed favorable outcomes. CONCLUSIONS: Mortality is very high in the first days after admission for primary spontaneous cerebral hemorrhage at Santa Rosa Hospital, with a probability of survival in the first 10 days of approximately 20% and a probability of survival at day 30 of less than 10%. Factors that are significantly associated with a high mortality rate are: Glasgow coma scale less than 8 points, midline deviation, ventricular invasion of the hemorrhage, and neutrophil-lymphocyte index greater than 4
