Background: Acute appendicitis is one of the most common pathologies within the acute abdomen that requires surgical resolution. Appendicitis that does not present surgical complications is in the catarrhal and suppurative phase, While the complicated ones, in the necrotic and perforated phase. The latter can be fatal. During the COVID-19 epidemic, an increase in the number of cases of complicated appendicitis will be observed among patients who suffered the viral infection, so our work sought to demonstrate an association between both diseases. Methods: Matched case-control study, in which clinical records of patients between 18 and 80 years of age who were operated for appendicitis, and who had at least one SARS-CoV-2 diagnostic test, were selected. Patients operated on for complicated appendicitis were considered cases, and patients operated on for uncomplicated appendicitis were controls. A 1:1 matching by sex, age, and month of surgery was performed. Conditional logistic regression modeling was performed to calculate the crude and adjusted Odds Ratio. Results: Cases had a 48.26% of COVID-19 positivity, as opposed to 20.90% of controls. A crude OR of 3.50 (95%IC 2.18 - 5.62, p = 0.001) was obtained, and an aOR of 4.12 (95%IC 1.83 - 9.27, p < 0.001) after adjusting for time of illness, waiting time for surgery, glucose, urea, creatinine and white blood cells. Conclusions: Being operated on for complicated appendicitis was associated with SARS-CoV-2 infection. This could be explained by the ability of the virus to infect gastrointestinal cells given the presence of the ACE2 receptor.
