Objective: To compare the frequency of local recurrence between patients with rectal cancer where “Watch and Wait“ (WW) was done and in patients operated on with a complete pathological response (pCR). Methods: Retrospective cohort study in patients with rectal cancer treated with neoadjuvant regimens where the exposure was the therapeutic approach of “Watch and Wait“ (WW), compared with the use of surgery with rPC. Recurrence was evaluated according to treatment approach and survival function curves were constructed using the Kaplan Meier method. Results: Of the 128 patients, 28 had a local recurrence of rectal cancer. The WW group and the surgery group had 14 patients each, similar behavior patterns were observed for both treatment approaches in the survival curves. It was found that recurrences in more than 50% were within the first 12 months after there was no trace of disease. Conclusions: The Watch & Wait approach in patients with complete clinical response (CRc) after neoadjuvant treatment does not show statistically significant differences in the development of local recurrences of rectal cancer compared to the surgical approach in patients with complete pathological response (PCR).
