To determine if excessive medication and self-medication are the pharmacological factors associated with the increase in migraine attacks in patients aged 18 to 65 years treated at the Hospital de Apoyo II-2 Santa Rosa, Piura. Period 2020-2023. Material and methods: Retrospective analytical observational cohort study in which 128 patients with a diagnosis of migraine who attended the Hospital de Apoyo II-2 Santa Rosa de Piura during the period 2020-2023 were included, who met the established inclusion criteria. A survey validated by experts was applied by telephone. The data were processed in STATA v.18.5 statistical software. An estimator of the magnitude of the relative risk association was calculated, reporting its 95% confidence intervals. Results: In the study, 88.28% of participants were female. Among patients with increased migraine attacks, the frequency of female sex was higher (94.92%) compared to male sex (5.08%), (p = 0.03). Among participants who reported self- medication, headache duration of 1 day (42.19%) was significantly more frequent (p = 0.001), compared to participants without self-medication. Among participants who reported excessive medication, headache duration greater than 1 day (54.55%) was significantly more frequent (p = 0.004), compared to patients without excessive medication. It was found that the variables independently associated with an increase in migraine attacks were self-medication (RRa 1.78, 95% CI 1.21 - 2.63, p = 0.004) and excessive medication (RRa 1.98, 95% CI 1.44 - 2.71, p = 0.000). In addition, it was identified that the consumption of paracetamol (RRa 1.69, 95% CI 1.16 - 2.44, p = 0.005), NSAIDs (RRa 1.57, 95% CI 1.10 - 2.24, p = 0.013) and ASA (RRa 1.63, 95% CI 1.14 - 2.33, p = 0.007) were variables independently associated with the increase in migraine attacks
