Abstract: Determinar si la medicación excesiva y la automedicación son los
factores farmacológicos asociados al aumento de crisis migrañosa en pacientes
18 a 65 años atendidos en el Hospital de Apoyo II-2 Santa Rosa, Piura. Periodo
2020-2023.
Material y métodos: Estudio observacional analítico retrospectivo de tipo
cohorte en el cual se incluyó 128 pacientes con diagnóstico de migraña que
acudieron al Hospital de Apoyo II-2 Santa Rosa de Piura durante el periodo 2020-
2023, que cumplían con los criterios de inclusión establecidos. Se aplicó una
encuesta validada por expertos vía telefónica. Los datos fueron procesados en
el software estadístico STATA v.18.5. Se calculó estimador de la magnitud de
asociación riesgo relativo, reportando sus intervalos de confianza al 95%.
Resultados: En el estudio, el 88.28% de participantes eran de sexo femenino.
Entre los pacientes con aumento de crisis migrañosa, la frecuencia del sexo
femenino fue mayor (94.92%) comparado con el sexo masculino (5.08%), (p =
0.03). Entre los participantes que reportaron automedicación, la duración de la
cefalea de 1 día (42.19%) fue significativamente más frecuente (p = 0.001),
comparado con los participantes sin automedicación. Entre los participantes que
reportaron medicación excesiva, la duración de la cefalea mayor a 1 día (54.55%)
fue significativamente más frecuente (p = 0.004), comparado con los pacientes
sin medicación excesiva. Se halló que las variables independientemente
asociadas a el aumento de crisis migrañosa fueron la automedicación (RRa 1.78,
IC95% 1.21 – 2.63, p = 0.004) y medicación excesiva (RRa 1.98, IC95% 1.44 –
2.71, p = 0.000). Además, se identificó que el consumo de paracetamol (RRa
1.69, IC95% 1.16 – 2.44, p = 0.005), AINES (RRa 1.57, IC95% 1.10 – 2.24, p =
0.013) y AAS (RRa 1.63, IC95% 1.14 – 2.33, p = 0.007) fueron variables
independientemente asociadas al aumento de crisis migrañosa.
Conclusiones: Los factores farmacológicos asociados al aumento de crisis
migrañosa en pacientes de 18 a 65 años son mediación excesiva y
automedicación
