Objective: To determine whether there is greater obstetric morbidity in pregnant women with pregestational diabetes compared to pregnant women with gestational diabetes at the José Cayetano Heredia Regional Hospital . Methods: An observational, retrospective, cohort and analytical work was performed. The study included pregnant women who were attended from 2014 to 2018 in the obstetrics and gynecology service of the Hospital José Cayetano Heredia, with a single pregnancy, with maternal age between 20-35 years and with legible clinical histories containing all the study variables. The sample consisted of 180 pregnant women divided into three groups (pregnant women without gestational diabetes without obstetric morbidity, pregnant women without diabetes with obstetric morbidity and pregnant women with gestational diabetes with obstetric morbidity). Statistical analysis was performed in Stata v14, using Poisson regression multivariate analysis to assess the association between variables. Results: The incidence of postpartum gestation was higher among pregnant women with pregestational diabetes than among pregnant women with gestational diabetes (28.89% vs 15.56%, p=0.019), similarly to the prevalence of postpartum hemorrhage (37.78% vs 13.33%, p=0.001). In multivariate analysis gestational diabetes was associated with 19% (RR=0.81, 95% CI:0.68-0.95) less relative risk of developing postpartum hemorrhage. Conclusions: A higher incidence of two of the six causes of obstetric morbidity was identified among pregnant women with pregestational diabetes compared with pregnant women with gestational diabetes and without diabetes.
