Objective: To determine if the elevated liver fibrosis index is a predictor of inhospital mortality in patients with decompensated congestive heart failure at the Hospital Belén de Trujillo. Material and methods: A retrospective, analytical and cohort study was carried out, in which 108 adult patients were included, according to selection criteria, which were divided into 2 groups: patients with a high or low FIB4 index; applying the relative risk, and the chi-square statistical test. Results: The frequency of mortality in patients with congestive heart failure and an elevated liver fibrosis index was 19/54 = 35%. The frequency of mortality in patients with congestive heart failure and an elevated liver fibrosis index was 7/54 = 13%. The multivariate analysis through logistic regression corroborates the superior statistical significance of the elevated FIB4 index with respect to intervening variables, among which hyponatremia, atrial fibrillation and metabolic acidosis stood out as predictors of in-hospital mortality in patients with congestive heart failure. (p <0.05). The practical significance of the elevated liver fibrosis index as a predictor of in-hospital mortality in congestive heart failure is accepted based on the similar results in previous investigations, its practical utility and theoretical acceptance. Conclusion: The elevated liver fibrosis index is a predictor of in-hospital mortality in patients with decompensated congestive heart failure at the Hospital Belén de Trujillo.
