Introduction: Acute appendicitis is a highly frequent clinical-surgical disease in pediatric patients. Despite the investigations carried out, there is controversy about the choice of the best surgical technique. Objective: Determine the effectiveness of laparoscopic and open appendectomy by comparing the frequency of postoperative complications and hospital stay in pediatric postoperative patients with uncomplicated acute appendicitis. Materials and Methods: a retrospective cohort study was conducted, evaluating 225 pediatric patients seen in the period 1990-2017. The risk of presenting postoperative complications was determined through the RR. Statistical analysis was carried out in the SPSS version 23 program. Results: We studied 225 patients, 128 (56.9%) underwent laparoscopic appendectomy and 97 (43.1%) open appendectomy. The average age in the laparoscopic appendectomy group was 10.96 ± 3 years and in the open appendectomy group was 12.41 ± 3.1 years. In the laparoscopic appendectomy group, 57.8% were female, while in the open appendectomy group, 52.6% were male. The laparoscopic technique required a longer time in the operating room (101.87 ± 23.5 min vs 61.1 ± 17.2 min, p = 0.001). Hospitalization time was longer in open appendectomy (4.85 ± 2.6 days vs 3.25 ± 1.7 days, p= 0.001). Complications after surgery in the laparoscopic appendectomy group were significantly lower than in open appendectomy (26.6% vs 50.5%, p=0.02), such as intraabdominal abscess 3.9% vs 9.3%, operative site infection 6.3% vs 7.2% and postoperative ileus 16.4% vs 34% respectively. Conclusions: Laparoscopic appendectomy is more effective than open appendectomy in reducing the frequency of complications and postoperative hospital stay.
