Abstract: Evaluar si la edad, el sexo, el tipo de trauma, el nivel de lactato
en sangre, el RTS, el AIS, las lesiones asociadas, el shock y las transfusiones
sanguíneas son predictores de complicaciones post laparotomía por trauma.
MATERIAL Y MÉTODO: Realizamos un estudio observacional, analítico,
de casos y controles anidados en una cohorte, que evaluó 65 pacientes operados
por trauma abdominal, los cuales fueron distribuidos en dos grupos, los casos
(35 pacientes con complicaciones postoperatorias) y los controles (30 pacientes
sin complicaciones), atendidos en el Hospital Belén de Trujillo durante el
período de Enero del 2010 a Diciembre del 2014.
RESULTADOS: La edad promedio en el grupo de casos fue 28,31 ± 10,41 vs
30,20 ± 15,80 años en los controles; el 88,57% fueron varones en los casos
frente al 93,33% en los controles; en lo que respecta al tipo de trauma, el 5,71%
de los casos se debieron a trauma cerrado, frente al 26,67% en los controles.
Los factores asociados a complicaciones luego de laparotomía por trauma
fueron: el lactato sérico (1,93 ± 1,79 vs 1,05 ± 0,65; p < 0,05); el RTS (7,46 ±
0,73 vs 7,74 ± 0,20; p < 0,05); el AIS (3,23 ± 0,65 vs 2,43 ± 0,94, p < 0,001);
las lesiones asociadas (97,14% vs 76,67%, p < 0,05); el shock (37,14% vs
3,33%, p < 0,01); las transfusiones sanguíneas (62,86% vs 13,33%, p < 0,001).
La mortalidad encontrada fue 2,86% en los casos frente al 0% en los controles
(p > 0,05).
CONCLUSIONES: El tipo de trauma, el nivel de lactato en sangre, el RTS, el
AIS, las lesiones asociadas, el shock y las transfusiones sanguíneas fueron
factores de riesgo predictores de complicaciones post laparotomía por trauma.
