OBJECTIVE: To determine whether in patients with suspected acute appendicitis the use of the Alvarado score associated with the ultrasonographic findings compared to the use of the Alvarado score there is only a lower rate of negative appendectomy at the Hospital Regional Docente de Trujillo during the period between January to December 2016. MATERIAL AND METHODS: We performed an observational, analytical, diagnostic test, which evaluated 102 patients with suspected acute appendicitis. RESULTS: The rate of negative appendectomies in the present study was 5.61%. The mean age for groups I and II were 37.21 ± 16.58 and 41.16 ± 12.54 (p > 0.05); the 51.49% of the patients were male in group I and in group II 100% (p < 0.05). Ultrasonography reported the diagnosis of acute appendicitis in 63.37% in group I and 50% in group II (p > 0.05); Alvarado score with cut-off point 7, had the diagnosis of acute appendicitis in groups I and II of 79.21% and 33.33% (p < 0.05); When combined with the use of ultrasound and the Alvarado score with cut-off point 7, the diagnosis of acute appendicitis in groups I and II was found to be 53.46% and 16.67% (p = 0.08). The diagnostic performance of the combined ultrasound with the Alvarado score for acute appendicitis were: sensitivity 53.47%, specificity 83.33%; VPP 98.18% and VPN 9.62% and Alvarado score at a cut-off point 7 were 79.21% sensitivity, 66.67% specificity; VPP 97.56% and VPN 16%. CONCLUSIONS: The Alvarado score had only a higher diagnostic yield for acute appendicitis than with ultrasonography.
