To demonstrate that early globular packages transfusion is a risk factor for acute respiratory distress in patients with severe trauma at Regional Hospital of Trujillo syndrome during the period January to December 2014. MATERIAL AND METHODS: We conducted an observational, analytical, case-control study evaluating 132 patients ≥ 18 years of age with severe trauma, which were divided into two groups, 44 patients with ARDS cases and controls 88 patients without ARDS. RESULTS: The mean age of cases was 32.34 ± 14.69 years and the controls was 31.45 ± 12.79 years; the proportion of male gender in cases and controls was 93.18% and 88.64% respectively. Blunt trauma was observed in 20.45% and 18.18% in cases and controls respectively. The RTS and hematocrit level in cases was 7.47 ± 0.76 and 29.40 ± 5.27%, and the controls was 7.72 ± 0.40 and 37.21 ± 5.27% (p < 0.001) .With respect to the transfusion of packed red blood cells within the first 24 hours, we found that 79.55% of the cases and controls 26.14% as received (p < 0.001); the mean number of transfused packed red blood cells was 3.71 ± 1.96 vs 2.13 ± 0.82 units (p <0.01). In regard to the need for ICU (3.41% vs. 22.73%, P <0.001) and mortality (6.82% vs. 0%; p <0.05) in cases and controls respectively. CONCLUSIONS: Early transfusions of packed red blood cells and the number of them were associated with the development of acute respiratory distress syndrome (ARDS). The severity of the trauma is a predictor independent of risk for ARDS's appearance.
