To determine if the surgical Apgar score is effective as a predictor of complications in patients undergoing emergency open abdominal surgery. Materials and methods: An observational, analytical-retrospective, cross- sectional study was carried out. Patients undergoing emergency open abdominal surgery were studied in the general surgery department of the Regional Teaching Hospital of Trujillo, during January to June 2022. Results: Age, type 2 diabetes mellitus, ASA classification, lowest mean arterial pressure, lowest heart rate, blood loss and operative time were statistically significantly associated (p<0.05) with the development of postoperative complications. 15 patients developed postoperative complications in group I, 1 patient in group II, 7 in group IIIB and 2 in group IVB. The area under the curve as a measure of the performance of the surgical Apgar score to predict the development of postoperative complications, showed a value of 0.850 with a significant level of p<0.001 with 95% CI (0.759-0.940). Conclusions: The prevalence of complications in patients undergoing emergency open abdominal surgery was 17.5%. The low-risk surgical Apgar was 4.2%, with moderate risk it was 9.1% and with high risk it was 4.2%. Age, diabetes mellitus and ASA classification and all intraoperative intervening variables had a significant association.
