To determine if the neutrophil/lymphocyte ratio (NLR) is a predictor of survival in women diagnosed with triple negative breast cancer (TNBC), treated at the “Hospital III-I José Cayetano Heredia – Piura”. Material and methods: Retrospective cohort analytical study in patients with TNBC treated at the “Hospital III-I José Cayetano Heredia – Piura”, during the period 2014 to 2019. Through convenience sampling, 96 patients were included, divided into two groups with NLI. high or low (cut-off point: 1.70). Overall survival (OS) and disease-free survival (DFS) were evaluated using Kaplan Meier curves. The intervening variables were evaluated with the Hazard Ratio (HR) with a 95% confidence interval. Results: The mean follow-up time until the first recurrence was 42.15 months and the mean until death was 64.73 months. Five-year OS in patients with high NLR was worse compared to low NLR (52.03% vs 87.99%; p < 0.001). The five- year DFS in patients with high NLR was lower compared to patients with low NLR (34.99% compared to 50.67%, p = 0.03). In the multivariate regression analysis, a worse overall survival prognosis was identified in those patients with a high NLR (aHR 4.0; 95% CI: 1.60-9.97). Conclusions: An NLR value greater than 1.7 is associated with shorter five-year OS and early DFS
