Complicated acute appendicitis represents a clinical challenge, and choosing the appropriate surgical method is crucial to optimize postoperative outcomes in the pediatric population. Objective: Determine if the Laparoscopic appendectomy is more effective than open appendectomy in pediatric patients with complicated acute appendicitis. Materials and methods: It is an analytical, retrospective, cohort study. 200 patients will be included (100 within cohort 1 and 100 within cohort 2) based on inclusion and exclusion criteria. The proposed collection includes the selection of medical records of patients from the Lazarte Hospital in Trujillo. The data collection and information organization instrument will then be filled out. Results: There is a statistically significant association between the type of appendectomy and two postoperative outcomes: days of hospitalization (Coefficient β -0.85 [95% CI -1.03 to -0.67], p<0.001) and surgical site infection (Relative risk 0.31 [CI 95% 0.1 - 0.91], p=0.034). That is, pediatric patients who underwent laparoscopic appendectomy compared to those who underwent open appendectomy had fewer days of hospitalization (4 vs. 5 days) and a lower incidence of surgical site infection (23.5% vs. 76.5%). Conclusions: In the present investigation we can conclude that laparoscopic appendectomy is more effective than open appendectomy, with respect to days of hospitalization and surgical site infection.
