Resumen: Objetivo: Determinar si la hidronefrosis representa un predictor para
apendicitis complicada, en pacientes con apendicitis aguda.
Material y métodos: Se plasmó un estudio observacional, transversal
analítico y prospectivo; que incluyó a 145 pacientes con diagnóstico
histopatológico de apendicitis aguda, pertenecientes al Servicio de Cirugía
del Hospital Regional Docente de Trujillo de junio a diciembre del 2022. Estos
fueron clasificados en dos grupos según resultados de patología: pacientes
con apendicitis aguda complicada y no complicada. Se revisó los resultados
de ultrasonografía abdominal evaluando la presencia o ausencia de
hidronefrosis. Los datos fueron ingresados a un análisis estadístico utilizando
medidas de tendencia central y de dispersión y prueba de chi cuadrado,
considerando el valor de P < 0.05 estadísticamente significativo.
Resultados: Se encontró que 95/145 (65.5%) pacientes tuvieron diagnóstico
histopatológico de apendicitis aguda complicada y 50/145 (34.4%) apendicitis
no complicada. De los pacientes con apendicitis aguda complicada, el 6.32%
(6 pacientes) presentaron hidronefrosis derecha vs el 2% (1 paciente) en el
otro grupo; lo cual no representó una asociación estadísticamente
significativa. El tiempo operatorio se identificó como factor asociado (p< 0.05)
a la apendicitis aguda complicada, estableciéndose un promedio de 71.34 ±
32.20 minutos vs 52.16 ± 21.16 minutos, para apendicitis aguda complicada
y no complicada respectivamente.
Conclusión: La presencia de hidronefrosis derecha no es un predictor de
apendicitis aguda complicada; sin embargo, su presencia es clínicamente
relevante

The results of abdominal ultrasonography were reviewed, evaluating the presence or absence of right hydronephrosis. The data was entered into a statistical analysis using measures of central tendency and dispersion and chi square test, considering the value of P < 0.05 statistically significant. Results: The present study found that 95/145 (65.5%) patients had a histopathological diagnosis of complicated acute appendicitis and 50/145 (34.4%) patients had uncomplicated appendicitis.
