To determine the effectiveness of mesh fixation with absorbable versus non-absorbable suture in inguinal hernia repair in patients operated on with the Lichtenstein technique. Material and methods: We conducted an analytical, cohort, retrospective study. A total of 200 medical records of inguinal hernia postoperative patients were reviewed at the Trujillo´s Regional Teaching Hospital during the period January 2016 - January 2019, 78 patients who met the inclusion criteria were selected, these patients became part of the following exposure groups according to the type of suture used for mesh fixation in inguinal hernioplasty: Cohort I (27 patients) if they used absorbable suture. Cohort II (51 patients) if they used nonabsorbable suture, in them the incidence of postoperative complications was compared, such as: hematoma, seroma, chronic pain and hernia recurrence. Results: 7% of the patients who were underwent mesh fixation in inguinal hernia repair had chronic groin pain, 21% had it with non-absorbable suture fixation, a lower incidence of chronic groin pain is evidenced using absorbable suture, being statistically significant RR of 0.41 (CI 0.2-0.8). Regarding the complication rates of hematoma, seroma and inguinal hernia recurrence, no significant differences were found (p < 0,05) between the use of both sutures. The predominant place of presentation of inguinal hernia was the left side and the predominant type was indirect. The mean age was 61 and 60 years for the absorbable and non-absorbable exposure group correspondingly. Conclusions: The use of absorbable suture in mesh fixation in inguinal hernia repair is associated with a lower rate of chronic groin pain compared with non-absorbable suture. The percentages of hematoma, seroma and recurrence of inguinal hernia presented in both sutures used were not significant
