To identify the risk factors for exchange transfusion in neonates with severe neonatal hyperbilirubinemia treated at the Víctor Lazarte Echegaray Hospital in Trujillo during the period 2010-2018. MATERIALS AND METHODS: A case-control, analytical, retrospective, longitudinal and observational study was carried out. The study population consisted of neonates with severe neonatal hyperbilirubinemia hospitalized in the Neonatology service of the Víctor Lazarte Echegaray Hospital in Trujillo, in the period from January 2010 to December 2018. The cases were those that required exchange transfusion, excluding those who presented associated diagnosis of polycythemia, direct predominant hyperbilirubinemia, premature and low birth weight patients; The controls were those in phototherapy that did not require exchange transfusion, with similar exclusion criteria as in the cases. RESULTS: 206 patients were studied. Of which 103 were cases and 103 controls. Among the risk factors studied: ABO incompatibility had an OR = 1.79; with a value of p <0.05 and a 95% confidence interval (CI). Regarding cesarean delivery, an OR = 2.4; with p <0.05, 95% CI. Regarding Rh incompatibility, OR = 1.36; p value> 0.05, 95% CI. With respect to Positive direct Coombs test, OR = 1.26; p> 0.05, 95% CI. Regarding male sex, an OR = 1.22 was obtained; p value> 0.05, 95% CI. Regarding sepsis, OR = 1.11; p> 0.05, 95% CI. Apgar less than 7, OR = 1; p = 1, 95% CI. Regarding hypertensive disease of the mother, OR = 1; p-value = 1, 95% CI. CONCLUSIONS: ABO incompatibility and cesarean delivery are risk factors for exchange transfusion in neonates with severe neonatal hyperbilirubinemia in the present study.
