We present the case of a 24-day-old male neonate with frequent and intense vomiting, which is taken to several health centers (health ministry and private) without a definitive diagnosis and without timely treatment, generating exacerbation of the symptoms, triggering severe dehydration, so he is referred to an EsSalud center where he undergoes auxiliary tests showing hypoglycemia, hyponatremia, hypocalcemia, which is why congenital adrenal hyperplasia is suspected and he is given a dose of 17-OHpregnenolone with positive result for the disease The technical standard NT 154-MINSA / 2018 / DGIESP of Health for the neonatal screening of congenital hypothyroidism, congenital adrenal hyperplasia, phenylketonuria, cystic fibrosis, congenital hearing loss and congenital cataract approved by RM No. 558-2019-MINSA establishes to carry out neonatal screening for all newborns at all levels of care, whether through essalud and minsa; In this case, neonatal screening was performed; but the delay in delivering the results in this patient was an important point since early treatment had been initiated and morbidity and mortality in this patient had been avoided.
