Abstract: La Hiperplasia Suprarrenal Congénita (HSC) engloba a un grupo de trastornos
hereditarios de la esteroidogénesis suprarrenal de colesterol; de los 5 tipos, la forma más
común es el déficit de la 21 hidroxilasa con 95% de casos.
En el déficit de 21 hidroxilasa encontramos dos formas de presentación: clásica que cursa
con hiperandrogenismo y condiciona macrogenitosomia en varones y virilización en
mujeres, y la forma perdedora de sal con crisis de pérdida salina aguda y gran mortalidad
de no instaurarse el tratamiento precozmente.
Presentamos el caso de un neonato varón de 24 días de edad con vómitos frecuentes e
intensos, quien es llevado a varios centros de salud (Ministerio de Salud y privados) sin
que se defina un diagnóstico, generando exacerbación de los síntomas, desencadenando
un cuadro de deshidratación moderada a severa, por lo que es derivado a un centro de
EsSalud donde le realizan exámenes auxiliares evidenciándose hipoglucemia,
hiponatremia, hipocalcemia, por lo que se sospecha de hiperplasia suprarrenal congénita
y se le realiza dosaje de 17-OH-pregnenolona con resultado positivo para dicha
enfermedad.
La norma técnica NT 154-MINSA/2018/DGIESP de Salud para el tamizaje neonatal de
hipotiroidismo congénito, hiperplasia suprarrenal congénita fenilcetonuria, fibrosis quística, hipoacusia congénita y catarata congénita aprobada por RM Nº 558-2019-
MINSA establece realizar el tamizaje neonatal a todos los recién nacidos en todos niveles
de atención ya sea por el seguro social de salud (EsSalud) y el Ministerio de Salud
(Minsa); en este caso se realizó el tamizaje neonatal después de las 48 hrs de vida, no
obstante hubo un retraso en la entrega de resultados, generando así un riesgo potencial
para su vida; este hecho fue un punto importante ya que se pudo iniciar un tratamiento
precoz y evitar la morbimortalidad de este paciente.
