Abstract: Demostrar que el índice neutrófilo/linfocito (INL) es útil como marcador
de mortalidad en pacientes con cirrosis hepática (CH).
MÉTODO: Estudio de pruebas diagnósticas, que incluyó a 138 pacientes
hospitalizados por CH descompensada en el Hospital Belén de Trujillo, 2017-2019;
en quienes se calculó el INL al ingreso hospitalario y se evaluó su mortalidad
intrahospitalaria. Se determinó el punto de corte, área bajo la curva ROC,
sensibilidad, especificidad, valor predictivo positivo (VPP) y negativo (VPN) del INL.
RESULTADOS: El 32.6% (45 pacientes) había fallecido por CH, cuya edad
promedio fue de 65.47 años de los cuáles un 53.3% era del sexo masculino. La
clase C de Child-Pugh se asoció a mayor mortalidad (p<0.001), al igual que haber
ingresado a hospitalización por encefalopatía hepática (p=0.001) o por hemorragia
digestiva alta (p=0.015). El punto de corte del INL fue de 3.7 (área bajo la curva de
0.775, p<0.001), que presentó una sensibilidad de 86.7%, especificidad de 58.1%,
VPP de 50% y VPN del 90% como marcador de mortalidad por CH.
CONCLUSION: el INL es útil como marcador de mortalidad en pacientes con
cirrosis hepática.
To demonstrate that the neutrophil/lymphocyte ratio (NLR) is useful as a marker of mortality in patients with liver cirrhosis (LC). Method: Diagnostic test study, which included 138 patients hospitalized for decompensated LC at the Hospital Belén de Trujillo, 2017-2019; in whom the INL was calculated at hospital admission and their in-hospital mortality was evaluated. The cut-off point, area under the ROC curve, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of INL were determined. Results: The 32.6% (45 patients) had died for LC, whose average age was 65.47 years and made up of 53.3% males.
