Abstract: La intolerancia alimentaria en bebes pretérmino, propia de su inmadurez digestiva, constituye un
gran reto en relación con el inicio, progresión e interrupción de la alimentación enteral. Las
recomendaciones con respecto a los requerimientos nutricionales en bebés prematuros se han
realizado con la finalidad de lograr un crecimiento y desarrollo óptimos para prevenir daños
asociados tanto a déficit o como a excesos de nutrientes. Así mismo, la estandarización de
protocolos que determinen las condiciones para el inicio de la alimentación enteral con lactancia
materna exclusiva, su volumen de progresión y su fortificación se basan no solo en sus efectos
benéficos sino en contribuir a la disminución de la temida enterocolitis necrotizante (NEC). Esta
revisión presenta una síntesis de literatura sobre las características clínicas y los factores de
riesgo de intolerancia alimentaria en recién nacidos prematuros, especialmente los de muy bajo
peso al nacer (MBPN); así como, evidencia científica de la utilidad de la evaluación cualicuantitativa del residuo gástrico.
Food intolerance in preterm babies, typical of their digestive immaturity, constitutes a great challenge in relation to the initiation, progression and interruption of enteral feeding. The recommendations regarding the nutritional requirements in premature babies have been made in order to achieve optimal growth and development to prevent damage associated with either a deficit or excess of nutrients. Likewise, the standardization of protocols that determine the conditions for the initiation of enteral feeding with exclusive breastfeeding, its volume of progression and its fortification are based not only on its beneficial effects but also on contributing to the reduction of the dreaded necrotizing enterocolitis (NEC). This review presents a synthesis of the literature on the clinical characteristics and risk factors for food intolerance in preterm infants, especially those with very low birth weight (VLBW); as well as scientific evidence of the usefulness of the qualitative quantitative evaluation of gastric residual.
