Objective: To determine if the compression of the uterus suture increases the effectiveness of misoprostol in the prophylaxis of postpartum abdominal hemorrhage in patients treated at the Hospital Belén de Trujillo. Material and Methods: A prospective observational, analytical, cohort study was carried out between May 2019 and January 2020. 126 pregnant women undergoing abdominal labor entered the study, group A made up of 63 patients received compressive suture, plus misoprostol 400 µg rectally at cesarean delivery. Group B of 63 patients received misoprostol 800 µg sublingual ground intraoperatively, after removing the placenta. In both groups, the pre and postoperative hematocrit was evaluated. And the needs of uterine massage or ergometrine as a complementary treatment. The analysis of the differences was performed with the Chi Square test for proportions with significant association if p <0.05. Results: The use of sublingual misoprostol has a similar effectiveness in reducing the frequency of anemia in pregnant women who are pregnant, as the use of compression suture plus transrectal misoprostol (p> 0.05). The decrease in hematocrit greater than 6% in pregnant women under cesarean section is less in the group with sublingual misoprostol (p <0.05). No significant differences were found between both groups in the use of complementary measures such as uterine massage or ergotamine use, and in the need for blood transfusion (p> 0.05). Conclusions: The use of sublingual misoprostol has similar effectiveness compared to performing a compressive uterine suture associated with misoprostol transrectally in the prophylaxis of postpartum abdominal hemorrhage in patients treated at the Hospital Belen de Trujillo. Compressive suturing of the uterus does not increase the effectiveness of misoprostol in prophylaxis of postpartum abdominal hemorrhage.
