The objective of this stufy is to analyze the efficacy of descompressive craniectomy before and after 48 hours for the clinical improvement of those patienst who have suffered taumatic brain injury. MATERIALS AND METHODS: An analytical, observational, restrospective study was carried out. In the present study, a total of 62 patients were included, 21 patients with early descompressive craniectomy (EDC) less tan 48 hours were selected, who corresponded to the exposed group. In addition, 41 patients with CD (descompressive craniectomy) after 48 hours corresponding to the non – exposed group were selected. RESULTS: Patients who underwent early descompressive craniectomy < 48 hours, had an observed prevalence of no clinical improvement 3.5 times higher than that of patients who underwent descompressive craniectomy > 48 hours. It is found that there is a clinical improvement 57.1% in patients with early descompressive craniectomy, while those who did not improve were only 42.9%. while in patients undergoing late descompressive craniectomy (> 48 hours) a clinical improvement of 87.8% is observed compared to 12.2% of those who did not observe clinical improvement; this implies a statistically significant difference (p < 0.005), according to the chi square test. CONCLUSIONS: The PDC performed after 48 hours has better results specifically in patients < 65th age, the PDC intervention less tan 48 hours in those patients with ICP.
