Objective: To analyze the survival of patients with chronic kidney disease (CKD) according to clinical conditions at the beginning of hemodialysis. Method: The type of study was observational, analytical, of a retrospective cohort of 180 patients with CKD who started hemodialysis at the Victor Lazarte Echegaray hospital. The clinical conditions evaluated were metabolic acidosis, Hyperkalemia, anemia, hypoalbuminemia, GFR ≥ 10 mL/min/1.72 m2, Diabetes Mellitus 2, Arterial hypertension and type of vascular access, the outcome evaluated was all-cause mortality in a follow-up period of 48 months. Bivariate analysis, multivariate analysis and survival analysis were performed using the Kaplan-Meier method considering significant differences if the p-value < 0.05. Results: The initial clinical conditions associated with all-cause mortality were age ≥ 60 years (HR: 1.829, 95% CI: 1.142-2.928; p= 0.012) and metabolic acidosis (HR: 2.211, 95% CI: 1.234-3.962; p=0.008), The cumulative probability of survival at 12 months was 83%, at 24 months was 66%, at 36 months was 57% and at 48 months was 51%. The median survival time of deceased patients was 15 months (ICER: 20) and of surviving patients 33 months (ICER: 32) (p= 0.000). Conclusions: The clinical conditions at the start of hemodialysis that affected survival of patients with CKD were age ≥ 60 years and metabolic acidosis
