Objective: To compare the clinical outcomes of treatment with amebicide alone versus the combination of amebicide plus percutaneous needle aspiration in patients with uncomplicated amebic liver abscess. Materials and Methods: A systematic review was conducted, including original and primary randomized clinical trials found in databases such as PubMed, Scopus, Embase, Web of Science, and Google Scholar until October 2023. Various outcomes were analyzed, including the resolution rate of symptoms, duration of hospitalization, incidence of complications, mortality, and the time to radiological resolution of the abscess. The risk of bias in the included studies was assessed, and a fixed or random-effects model was used depending on heterogeneity. Results: The review included 8 randomized clinical trials. A higher rate of symptom resolution was observed in patients treated with amebicide plus percutaneous aspiration compared to those treated with amebicide alone (RR 1.14, 95% CI [1.00 - 1.29], p = 0.05). However, no significant differences were found in the incidence of complications, mortality, or time to radiological resolution of the abscess between the two groups. The duration of hospitalization showed a non-significant mean difference of -0.31 days (95% CI [-1.81 – 1.20], p = 0.69), but significant heterogeneity was observed (I2 of 80%). Conclusion: Although the combined treatment of amebicide and percutaneous aspiration showed a higher rate of symptom resolution in patients with uncomplicated amebic liver abscess, no significant differences were found in other clinical outcomes such as complications, mortality, time to radiological resolution, or hospitalization duration
