Background: Cesarean section is one of the most performed surgical procedures worldwide, the number of women who have completed pregnancy through caesarean section has increased in recent years. Cesarean section should only be performed when there are indications to improve the well-being of the pregnant woman, the newborn and improve the outcome. The International Federation of Obstetrics and Gynecology (FIGO) suggested not exceeding the C-section rate of 15% at its annual meeting in Vancouver in 2015. Methods: The research design is observational, longitudinal, analytical, being a retrospective case-control study, the sample consisted of “143” full-term pregnant women with a previous cesarean section at the Guillermo Diaz de la Vega Hospital, and “281” in the Adolfo Guevara Velasco hospital, fulfilling the inclusion criteria observed in the Obstetrics area of both hospitals during the years 2019-2020. Expected results: A cesarean section rate was obtained at the Guillermo Diaz de la Vega Hospital in Abancay of 42%, and 40% at the Adolfo Guevara Velasco Hospital. Furthermore, a vaginal delivery success rate of 32.9% and 33.1% was obtained in both hospitals respectively. Associated factors were: previous vaginal deliveries: (Wald: 53.2; P: 0.000, OR 0.38) in Cusco, and (Wald: 20.9; P: 0.000, OR 3.47) in Abancay; parity: (Wald: 52.4; p 0.000, OR: 4.22) in Cusco and Abancay (Wald: 20.93; p 0.000, OR: 4.17); gestational age (Wald: 29.2; p 0.000, OR 3.3) in Cusco, while in Abancay there was a (Wald: 17.68; p 0.001, OR 5.98). The fetal weighted: (Wald: 31.26, p 0.000, OR: 1.8) in Cusco, in Abancay (Wald: 7.12, p 0.029, OR: 1.63). On the other hand, the main cause of previous caesarean sections in both hospitals was pelvic brain disproportion, being 32.2% in Abancay and 37% in Cusco. As there were also more early complications in Abancay than in Cusco, the most frequent being grade I tear Conclusions: It is concluded that there are prognostic factors such as a history of previous vaginal deliveries, parity, gestational age, and fetal weight that are associated with successful vaginal delivery in pregnant women with a history of previous cesarean section. Cephalopelvic disproportion is the main cause of previous cesarean section.
