Introduction: Acute appendicitis is one of the common causes of acute abdomen. The perforated appendicitis rate ranges from 20% to 30%; However, it can be as high as 50% in the elderly. Surgical treatment is essential in the management of appendicitis. The surgical arternatives are laparoscopic appendectomy and open appendectomy. Methods: Analytical observational study, where clinical results (operative time, hospital stay and operative site infection) were analyzed in adult patients undergoing laparoscopic appendectomy and open due to perforated acute appendicitis, given at the Victor Lazarte Echegaray Hospital between 2016 and 2018. Results: 160 medical records were reviewed. Women accounted for 45%, the average age was 38,3 years. 62,5% of the patients were operated with open appendectomy and 37,5% by laparoscopy. Surgery time when comparing AL: 70.5 ± 16.0 min vs. AA: 59.3 ± 12.0 min, had an average that was 56,38 minutes (SD: 22,68 minutes); The average number of hospitalization days was 4,75 (SD: 4,61) and 84 (53,85%) developed an infection of the operative site. Discussion / Conclusions: In our study, when comparing laparoscopic appendectomy vs open appendectomy, laparoscopic appendectomy is presented as the best alternative for a shorter hospital stay, a lower chance of infection, and less pain after the intervention. While open appendectomy is shown as a better option if shorter surgery time is sought.
