To determine which are the risk factors that increase the probability of postoperative mortality in elderly patients with acute cholecystitis at the Hospital de Alta Complejidad ""Virgen de la Puerta"" during the period from January 2015 to September 2022. MATERIAL AND METHOD: An observational, retrospective study of Cases and Controls was designed, where the sample to be studied were older adults of the fourth age, that is those who have 80 years of age, attended in the last 8 years (2015 - 2022) in the Hospital de Alta Complejidad Virgen de la Puerta, from this sample two groups were formed, the case group, those who died, and the control group, those who survived. Information was collected on sociodemographic, clinical, laboratory, surgical and postoperative complications variables, which were the exposure variables, and the resulting variable was mortality. In the statistical analysis we used the Chi-square test (X2), Fisher's test for categorical variables and Student's t-test for quantitative variables, with a statistical difference of less than 5% (p < 0.05) as well as the Odds ratio. RESULTS: In the bivariate analysis, the time of disease, the degree of cholecystitis, the ASA level and Diabetes mellitus were associated with postoperative mortality, but in the logistic regression analysis, Diabetes mellitus no longer had an association and the patients who presented grade III of acute cholecystitis had a 4.33 times higher risk of mortality compared to the group of patients with grade II (ORa 4.333 (95% CI: 1.701- 11.038) p<0.05). Patients with disease time >58 hours had a higher risk of dying compared to patients with shorter disease time (ORa 1.013 (95% CI: 1.003–1.023) p<0.05). Patients with ASA III had a higher risk of death compared to patients with ASA II (ORa 12.166 (95% CI: 11.477–12.855) p<0.05). CONCLUSIONS: Time of illness, degree of acute cholecystitis and ASA level of fourth-age patients with acute cholecystitis if were risk factors for postoperative mortality.
