The objective of the research was to determine the relationship between adverse drug reactions and polypharmacy in hospitalized patients of the Barranca - Cajatambo hospital in the period September - December 2021. The study was descriptive, cross-sectional, correlational and retrospective. Documentary analysis and the data collection form were used. ADRs are very significantly related to polypharmacy, especially with the number of ADRs, most affected organs and systems, therapeutic groups and types of ADRs according to Rawlins and Thompson with the prevalence of polypharmacy in (p=0.00, 69.7%, p=0.041 and p=0.008) respectively. The most affected organs and systems were the gastrointestinal system with 68.8%, the therapeutic groups causing ADRs were antibiotics with 52.5% and the most notorious type of adverse drug reactions according to the Rawlins and Thompson classification was type A with 53.7%
