The delays in reaching an appropriate health center and receiving care faced by women with complications related to pregnancy, childbirth and the puerperium, play an important role in the occurrence of these deaths. These delays are: 1) Lack of recognition of warning signs during pregnancy or the puerperium. 2) Delay in making the decision to seek care in health facilities. 3) Delay in accessing care and lack of logistics for the referral. 4) Inadequate quality of care. That is why the present work aims to describe the four delays that cause maternal death between the years 2012-201. For this, a descriptive and retrospective study was carried out, considering the total number of cases of maternal deaths registered by epidemiological surveillance in the “Regional Directorate of Loreto” from 2012-2019, being a total of 232. Within the results, it was observed that delay 1 occupies the first place of the causes with 60.8% (141/232) of cases and delay 4 occupies the second place with 49.1% (114/232), delay 2 occupies third place with a frequency of 37.5% (87/232) and delay 3 ranks fourth with a frequency of 19.8% (46/232). It is concluded that delay 1 was the most frequent in the years under study.
