Background: Being part of the fire department involves the exercise of a dangerous profession. Given the circumstances in which firefighters perform they are 3.5 times more likely to suffer an injury in the workplace and 3.8 times more likely to suffer from musculoskeletal disorders compared to a private sector worker, and take more than four times the time in return to work as a private sector worker. Objective: To characterize the severity of injuries in the lower limbs and verify the association with various characteristics of the population, the event, and the injury itself. Material and Methods: A secondary type analysis of NFIRS-5 module of 2014 was carried out. We worked with 1297 fire event records from the United States. Data on events in which the age group was 18 to 65 years old, with lower limb injuries, was included. Incomplete event records were excluded. The database was provided by "National Fire Incident Reporting System (NFRIS)" and was obtained through the Enigma Public page. These data was subdivided by their main characteristics: population, event, and injury. The response variable was the severity of injuries in lower limbs, valued from 1 to 7 (report, very mild, mild, moderate, severe, critical and death).The variables were analyzed descriptively, their bivariate association of factors with the severity and the strength of their association through the calculation of the Odd Ratio in a crude (ORC) and adjusted (ORA), for that manner the severity was dichotomized in two groups: severe group (severe and critical) and the report group (mild and moderate). Results: No significant associations was found with the variables gender, career, subject, state and physical condition. Fractures accumulate a higher percentage of severity (24.44%), followed by crush injuries (7.14%). Jumping accumulates a higher percentage of severity(10.53%), followed by exposure to danger (5.00%). In the same way, there is an association between the level of severity and the object where nature accumulates a greater percentage of severity and exposure to critical conditions(6.06%), followed by structural components (5.08%). For the symptom variable, significant values ORA > 1 were found at fracture with (ORA = 43), dislocation (ORA = 9.42) and crush injuries (ORA = 1.14). On the other hand, significant values of ORA <1 with sprain or contracture (ORA = 0.99), inflammation (ORA = 0.76), pain (ORA = 0.55), Abrasion (ORA = 0.30) Contusion (ORA = 0.27) and cut were found (ORA = 0.17). For the factor variable, significant values of ORA with Hole (ORA = 0.41) and Vehicle / Device (ORA = 0.23) were found. Conclusions: There is a high level of severity in the various reports which are associated with symptoms such as fractures and crush injuries. In the same way, surface conditions, weather and factors such as holes and vehicles mean greater danger for firefighters.
