Aim: to determinate the association between body mass index (BMI) at diagnosis and survival in treated patients with non-metastatic breast cancer. Methodology: Retrospective and correlational study in non-metastatic breast cancer. The recurrence free survival (RFS) and overall survival (OS) were determined according the BMI in overall patients and subgroups according clinical and pathological characteristics. Results: 1340 patients were studied. Median age was 52.4 [18-93]. 1.1% were underweight, 26.7% were normal BMI, 73.2 % were overweight, 28.1% were obesity and 29.8% were severe obesity. There was positive association between BMI ≥ 25 and postmenopausal status (p=0.001), and estrogen/progesterone receptors expression (p=0.04 and p=0.02; respectively); but a negative association with CerbB2 expression (p<0.05) and complete pathological response (p=0.01). The OS was lower in patients with low weight (p=0.02). The positive CerbB2 and negative hormone receptor breast cancer and postmenopausal with low weight showed lower OS that high BMI women (p=0.03 and p<0.05, respectively). There was not any significative difference in RFS between the BMI. The BMI not influenced on RFS or OS in any subgroup according the multivariate analysis. Conclusion: The BMI is associated with the OS in non-metastatic breast cancer patients, although not independently, underweight at diagnosis was a poor prognosis factor in non-metastatic breast cancer patients.
