Background: Anemia is a severe public health problem according to the WHO classification when the percentage of anemic is equal to or greater than 40%. In 2019, in Peru 40.1% of children are executed are anemic. This being a concern since, despite the state's effort to reduce the percent, since 2011 it has been stagnant. The treatment applied to deal with anemia is iron supplementation. Although iron is essential for the body, when it is in excess it can produce an adverse response, such as inflammation of the intestine and thus favor the proliferation of pathogens, inducing diarrhea. Objective: The main objective of the present study was to determine the association between diarrhea and iron supplementation in children with and without anemia, controlling for different sociodemographic. Material and methods: The DHS 2009 – 2017 database, which uses a complex sample design, was analyzed. The samples were weighted. Bivariate and multivariate statistical analyses adjusted by age, sex, geographical region, water and drainage tenure, rurality were carried out. The results are presented as a reason for prevalence. Results: Data show that iron supplementation in non-anemic children, at 12 months of exposure, increases the risk of diarrhea (p. 0.027); while drinking water and drainage services reduced the risk by almost 15% (p<0.05). Conclusions: Preventive iron supplementation would be causing an increase in the percentage of children who develop episodes of diarrhea.
