The main objective of the present study was to determine the relationship between sexual dimorphism and the presence of the fourth canal of the first permanent upper molar of patients from the private practice of the Artdental clinic using Cone Beam Iquitos 2019 tomography. The database was reviewed of the year 2019 selecting 182 CT scans which met the inclusion and exclusion criteria of 244 CT scans; The patients were selected according to sex, age group, and axial, sagittal and frontal cuts were made to verify the presence of the fourth canal, foramen and root of the Mesiobuccal root of the first permanent upper molar. The statistical program SPSS version 22 and the Pearson Chi2 square statistical test (p <0.05) were used. The results show that of the population studied, 182 tomographies 84 (46.2%) were male, 98 (53.8%) female, with a mean age of 41.05 ± 17.6 years. Of these, the percentage of the fourth canal was (74.7%) in the first permanent upper molar; being 79.8% in men and 70.4% in women, regarding its relationship with sexual dimorphism it had a value of p = 0.148; while the age group had a value of (p = 0.035) and with the maxilla side the right side had a value (p = 0.020), and the left side (p = 0.495) and in foramen and MV root it was seen that there is a relationship on the right side (p = 0.046), (p = 0.041). It is concluded that there is no relationship between sexual dimorphism and the presence of the fourth canal (p = 0.148), despite the high prevalence of 74.7% of the fourth canal.
