OBJECTIVE: To describe the level of response to bronchodilators in children under two years of age with a diagnosis of bronchiolitis and with a predictive index of positive asthma hospitalized in the pediatric service of the Iquitos Hospital "César Garayar García", during the period from July to December of 2017. MATERIALS AND METHODS: An observational, prospective and analytical study was designed that consisted of collecting information from patients diagnosed with bronchiolitis treated in the pediatric service of the Iquitos Hospital. The information was collected in a file created for this purpose, obtained from the parents, the patients and from the review of medical records that met the inclusion and exclusion criteria. RESULTS: A total of 30 patients with a diagnosis of bronchiolitis were analyzed, 14 of them had a positive predictive index of asthma (IPAm) and 16 predictive index of asthma (IPAm) Negative. These two groups were compared to evaluate the variables under study. Of the total patients 16 (53.33%) were female and 14 (46.67%) were male. The average age of the majority was between 0 to 6 months of age (63.3%). There were no comorbidities in either group and only one case of atelectasis was presented. The β-2 adrenergic agonists were the most commonly used Bronchodilator drugs for the treatment of bronchiolitis (83% of all patients), being equally distributed in both groups. The most prevalent major criterion of (IPAm) was atopic dermatitis (30%) and the most prevalent minor criterion was peripheral eosinophilia ≥4% (66.67%). Through the analysis it was possible to identify that the use of bronchodilators does not improve the clinical response in children under 2 years of age with a diagnosis of bronchiolitis, quantified with the Bierman and Pierson scale at 48 hours of admission (p= 0.458), nor at 72 hours, neither did the hospitalization time decrease (p=0.378), nor did it decrease the risk of oxygen need. CONCLUSIONS: Children under two years of age with a positive modified Predictor of Asthma (IPAm) index do not have a positive clinical response to the use of bronchodilators during the treatment of Bronchiolitis.
