Objective: To identify the management of open fractures of limbs in patients treated at the Department of Surgery of Regional Hospital of Loreto. July 2014 - June 2015. Methodology: A descriptive study, cross-sectional and retrospective, the sample consisted of all patients who presented diagnosis of open fractures of limbs, treated at the Regional Hospital of Loreto from July 2014 to June 2015, data generated from the review of medical records in the area of statistics at the same hospital. The convenience sample because it included all patients with the inclusion criteria in the study period. For the univariate analysis of data used descriptive statistics through scattering measurements, frequency and variance analysis. Results: From a total of 1052 patients treated in the Department of Surgery in the final year of study; July 2014 to June 2015; in the Regional Hospital of Loreto, 280 corresponded to the Traumatology and found that 38.6% (108 patients) were treated by open fractures of limbs, the male is the most frequent with 76.9% (83 patients). The average age is 38.9, with a range of 15-25 years 28.7% (31 patients) followed by the range of 26-35 years 22.2% (24 patients), 55.6% (patients) come from the urban area, the most representative is the marital status cohabitant with 44.4% (48 patients), the most common level of education was the secondary level with 47.2% (51 patients), the most common occupation was driver with 24.1% ( 26 patients), followed by the student with 14.8% (16 patients), as the most frequently affected limb is the left lower limb with 36.1% (39 patients), followed by the right leg with 23.1 (25 patients) The most frequent topographic location was the leg with 36.1% (39 patients) followed by the arm with 16.7% (18 patients), the mechanism was the direct production with 97.2% (105 patients), the most frequently affected bone was the tibia and fibula with 18.5% (20 patients), followed by the tibia and humerus with 16.7% (18 patients) each, the most frequent causes of open fractures found in our study were traffic accident in 40.7% (44 patients), followed by the Labour Accident with 22.2%, according to the classification of Gustilo and Anderson has the following distribution: Grade I (1.9%), grade II (32.4%), grade III (65.8%); the average hours the patient receives his first washed and debridement from suffering the fracture was 12 hours, 64.8% (70 patients) were cleaned and debrided before 6 hours, the average days of hospitalization was 23.5 days , being 70.4% (76 patients) from 11 to 30 days, the most common complication from the accident itself was infection with 43.5% (47 patients), followed by the main section of the neurovascular bundle with 38.9% and compartment syndrome in 22.2% (24 patients); and none 42.6% (55 patients) followed by infection with 35.7% (46 patients) derived from the treatment in our study 100% of patients received medical treatment with antibiotic, analgesic, topical antiseptics and splinting of plaster followed by 21.3% (23 patients) received blood transfusion; the most common surgical treatment was surgical cleaning with 100% (108 patients) and as a means of primary intramedullary nail fixation was 37% (40 patients) and external tutors with 25.9% (28 patients). Conclusions: The overall average age of patients with open fractures treated at the Regional Hospital of Loreto "Felipe Arriola Churches" in the period July 2014 to June 2015 is 38.9 years, age range 15-86 years, with the most frequent age group of 15-35 years with a 50.9% (55 patients) population that is economically active, male predominance with 76.9% (83 patients) on the female 23.1% (25 patients), in a relationship of 3-1, the incidence of the study is related to international statistics; with a notable increase in the cause of the fracture by traffic accidents with 40.7% open fractures of limbs were found more frequently in the tibia and fibula with 18.5% (20 patients), followed tibia and humerus with 16.7 % (18 patients) each anatomical region which is in relation to activities of the patients. In third place were open fractures of the foot bones with 13% (14 patients); washing and debridement was performed within the first 6 hours at a higher rate with 62.0% (67 patients), also stabilizing the fracture with 100%, according to current studies indicate that initial treatment should be conducted within the six hours with immediate stabilization, the long hospital stay is associated with the highest percentage of open fractures in large bones like tibia, fibula and humerus, and in many cases the surgical treatment consisted of cleaning and fixing materials, handling of open fractures of limbs involved multiple factors which include outpatient conditions (delay in transferring the patient), and nosocomial (socioeconomic status, availability of operating room, emergency service supersaturation, administrative factors) which may influence the appropriate patient, the highest percentage of open fractures of limbs are grade III with 68.9% (71 patients), which were located in long bones, followed by grade II with 32.4% (35 patients) and grade I with 1.9% (2 patients); 100% of patients with open fractures received medical treatment with antibiotics, analgesics, topical antiseptics and detention, and the most common surgical treatment was surgical cleaning with 64.8% (70 patients), followed by the placement of intramedullary nails with 31.5% (34 patients); in 100% of open fractures not sample for culture, which is essential to complete the proper treatment of these fractures was taken because even though antibiotic prophylaxis is put in place, there are many microorganisms that cause resistance to certain antibiotics.
