Purpose: To determine the usefulness of the neutrophil/lymphocyte index as a predictor of the development and severity of preeclampsia in pregnant women attended at the gynecology and obstetrics department of the Adolfo Guevara Velazco National Hospital Essalud-Cusco, 2017- 2019. Methods: Analytical, correlational, observational and retrospective case-control study. A total population of 322 pregnant women was obtained, comprising 161 cases and 161 controls, who met the inclusion criteria. The receiver operating characteristic (ROC) curve was used to find the cut-off point with the highest sensitivity and specificity of the neutrophil/lymphocyte index to predict the development of preeclampsia and the severity of preeclampsia. Simple logistic regression was used for qualitative variables and Pearson's Chi-square test for quantitative variables. Results: The cut-off point of the neutrophil/lymphocyte index for predicting the development of preeclampsia was found to be 3.08 (ROC=0.76), with a sensitivity of 65.8%, specificity of 78.9%, positive predictive value of 75.7% and negative predictive value of 69.8%. On the other hand, the cut-off point of the neutrophil/lymphocyte index for predicting preeclampsia with severity criteria was 3.74 (ROC=0.78), with a sensitivity of 51.3%, specificity of 95.7%, positive predictive value of 85.4% and negative predictive value of 79.8%. Conclusion: The neutrophil/lymphocyte index is useful as a predictor of preeclampsia and severity of preeclampsia in pregnant women.
