Introduction: Pathologies of the bile ducts caused by the presence of stones in the common bile duct can be diagnosed with different imaging tests, among which is endoscopic retrograde cholangiopancreatography (ERCP), which is considered the first option in management. endoscopic therapy of choledocholithiasis and other benign and malignant pathologies. Objectives: To determine the association of risk factors and complications of ERCP in patients diagnosed with choledocholithiasis at the Regional Hospital of Cusco from 2020 to 2022. Methodology: The present observational analytical study of cases and controls. The population was made up of those patients who presented complications after performing an ERCP. The sample consisted of 172 patients treated in the gastroenterology service of the Regional Hospital of Cusco in the period from May to December 2022. Results: The association of risk factors and complications: Female sex OR=3.221 IC [1.717–6.043] Pv=0.000, age less than 60 years OR=9.904 IC [0.123 – 0.550] Pv=0.002, Cholecystectomy OR=5.886 IC [2.802-2.364 ] Pv=0.000, Cirrhosis OR=2.592 IC [0.489– 13.744] Pv=0.221, Common bile duct diameter OR=0.382 IC [0.184-0.790] Pv= 0.0083, sphincterotomy OR=4.44 IC [2.107-9.356] Pv=0.00 0, Cannulation of the bile duct OR=13.569 IC [3.070-59.97] Pv=0.000, Stone extraction OR=0.038 IC [0.184-0.790] Pv=0.008. The percentage of complications of the ERCP procedure was 20.7%, of which 9.6% corresponded to pancreatitis, 4.13% to hemorrhage, 2.76% to cholangitis, and 0.68% to perforation. Conclusions: An association was found between the female sex, age less than 60 years, cholecystectomy, common bile duct diameter, sphincterotomy, bile duct cannulation, stone removal, with respect to post-ERCP complications.
