Objective: Compare the efficacy of thyroid ultrasound with cytological study of fine-needle aspiration biopsy in patients with thyroid nodules conducted at the Regional Hospital of Cusco from 2014 to 2019. Methods: The research uses a Descriptive, retrospective, cross-sectional, and observational study. Data collection was facilitated through a form containing general and clinical information from medical records. Data were processed using the statistical package SPSS v25. Results: The study predominantly includes females (96.8%), adults (61.7%), and individuals from rural areas (66%). The most common histopathological type is papillary thyroid carcinoma (38.3%). The most frequent cytological type is Bethesda II (27.8%), followed by Bethesda IV (24.1%). The most common thyroid ultrasound result in the study population is TR3 (33%). The Bethesda system, in relation to histopathology, has a sensitivity of 82.8%, specificity of 60%, and a kappa coefficient (0.434). In contrast, the TI-RADS ultrasound analysis has a sensitivity of 76.2%, specificity of 32.6%, and a kappa coefficient (0.086). Conclusions: The efficacy of thyroid cytological study according to the Bethesda system is a valuable diagnostic tool when suspecting a malignant thyroid nodule. Although the thyroid ultrasound system, TI-RADS, can detect a considerable number of positive cases, its low specificity can lead to significant clinical implications, including the necessity for invasive procedures. The comparison between TI-RADS and Bethesda demonstrates a moderate superiority of the latter in the detection of malignant cases within our study population.
