Introduction: exclusive breastfeeding over the years has proven to be essential for the adequate growth and development of infants under six months, so its cessation is related to future short and long-term consequences in the infant. Objective: Determine the factors related to the abandonment of exclusive breastfeeding in adolescent mothers and young adults in the hospitals of the city of Cusco, 2023. Methods: an observational, analytical, retrospective case-control study was carried out. with a total population of 143 mothers of infants with 6-12 months of age who had the inclusion and exclusion criteria, those who responded to a validated questionnaire which included the variables of the study. The abandonment of exclusive breastfeeding was calculated, as well as the association between the variables using the chi-square statistical test and the risk measurement was carried out using Odds Ratio (OR). Results: it was found that the factors associated with the abandonment of exclusive breastfeeding are: the preference for mixed breastfeeding (OR = 2.158), the lack of knowledge that it reduces respiratory and digestive allergies (OR = 5.192), the lack of knowledge that the breastfeeding generates safety (OR = 2.814), being single (OR = 2.649), occupation (OR = 4.390), cesarea (OR = 3.076), being primiparous (OR = 3.913), problems with the nipple (OR = 3.412), feeling of that the infant is not satisfied (OR = 3.884), perception of insufficient milk (OR = 3.642) and male gender (OR = 3.699). Conclusions: Regarding the factors related to the abandonment of exclusive breastfeeding, it was identified that the abandonment of exclusive breastfeeding was 39.1%. 11 associated factors were found that determined a greater risk of early abandonment of exclusive breastfeeding, so strategies aimed at this group of mothers must be determined.
