Introduction: The COVID-19 pandemic has generated morbidity and mortality in different vulnerable groups such as pregnant women, who go through physiological and immunological changes due to COVID-19 infection, where it was observed that a series of complications such as increased prematurity. It has been seen that at 2,377 meters above sea level there is no evidence of the association between COVID-19 in pregnant women and prematurity. Objective: Determine if COVID-19 in pregnant women is a risk factor for the development of prematurity treated at the Guillermo Diaz de la Vega Abancay Hospital 2020-2022. Material and method: Retrospective cohort study. Sampling was not random and a logistic regression was also performed for the following variables for convenience: prenatal care, interval between pregnancies, birth weight, maternal age, education, sex at birth, and occupation. Where the sample was 60 exposed (those pregnant women with a positive diagnosis of COVID-19 with a delivery starting at week 22) and 60 non- exposed (those pregnant women with a negative diagnosis of COVID-19 with a delivery starting at week 22) hospitalized in the obstetrics-gynecology service of the Guillermo Diaz de la Vega Abancay Regional Hospital in the period from 2020 to 2022. Results: An RR of 2.43 was found with a 95% CI [1.09 - 5.43] of p = 0.023. Furthermore, the moderate and severe degree of COVID-19 infection in pregnant women presented an RR of 4.36 with a 95% CI [2.60 - 7.33]. Conclusions: COVID-19 in pregnant women is a risk factor for the presence of premature birth at the Guillermo Díaz de la Vega Abancay Regional Hospital
