Introduction: Gastrointestinal cancer has a high incidence rate at local and national level, despite this, early detection and follow-up is ineffective and with poor prognosis being this a public health problem, In addition, there are no previous studies on the follow-up and the timely management of malignant lesions, so this study seeks to contribute to the management, monitoring and early detection of such lesions. Method: Non-experimental, analytical, correlational, cross-sectional and retrospective study where data collection sheets were used and for the statistical data analysis the SPSS program for Windows in version 25, for univariate analysis the use of descriptive statistics was through the distribution of frequencies with tables and graphs. For the bivariate analysis of categorical qualitative variables, chi square test was applied, considering a confidence level of 95% with a p<0.05. The study population consisted of 3423 patients, of whom 660 were included in the sample. Result: Of 660 patients, 550 were diagnosed with premalignant lesions where the female sex predominated (56%) from the age of 45 years and 110 patients with malignant lesions where the male sex predominated (54.55%) from the age of 45 years, according to endoscopy. Similarity was found in the procedence of the patients in both lesions, being more frequent in the urban area of Cusco and significant in the rural area of Anta. According to pathological anatomy, premalignant lesions predominated in 71% in the stomach, being the most frequent lesion the complete metaplasia, followed by colon (9%) and tubular adenoma as the most frequent finding. In the relationship between the endoscopic and anatomopathologycal diagnosis, the ulcer associated with chronic gastritis and the polyp to hyperplastic polyp predominated. An association was observed between chronic erythematous and erosive gastritis with complete metaplasia. Regarding patients with esophagitis, are related to Barrett's esophagus in its whole. In patients with malignant lesions according to endoscopy, the majority (53,6%) were confirmed by anatomopathology, with adenocarcinoma as the most frequent histological type (74,6% of those confirmed), especially the differentiated type. The most frequent location was in the stomach followed by the colon. Regarding the association of malignant lesions with their classification according to Vienna, Nakamura and the infiltrating type, the majority is unrepresentative due to insufficient data. The bivariate analysis of premalignant lesions found a Pearson chi-square value of 1138.950 and the likelihood reason of 717.683, both with 35 degrees of freedom and an asymptotic significance <0.001, which suggests that there is an association between endoscopic and anatomopathological diagnosis at the same as in malignant lesions where it was found that the Pearson chi-square value was 225.279 and the likelihood reason was 166.033, both with 20 degrees of freedom and an asymptotic significance <0.001, obtaining a lower result than 0.05, which indicates that this association is significant, so the hypothesis can be accepted, especially in malignant lesions due to its lower degree of freedom. Conclusion: Based on the analysis carried out, it is concluded that premalignant and malignant lesions show a strong relationship between endoscopic and anatomopathological diagnoses, being more significant in malignant lesions.
